PHYSICAL EXAMINATIONS

According to Illinois State Law, all students entering kindergarten and sixth grade must present evidence of a physical examination.  Dental examinations are required for kindergarten, second, and sixth grade students.  New students coming from out of state must have a physical before entering school.  Students who do not have their physical examinations and immunizations completed or do not have appointments for them will not be allowed to attend school.  Examination forms are available from the school office.  Immunization requirements can be obtained from the school nurse.
